RESUMO

Anaemia is amongst the major complications of malaria, a major public health problem in the Amazon Region in Latin America. We examined the haemoglobin (Hb) concentrations of malaria-infected patients and compared it to that of malaria-negative febrile patients and afebrile controls. The haematological parameters of febrile patients who had a thick-blood-smear performed at an infectious diseases reference centre of the Brazilian Amazon between December 2009-January 2012 were retrieved together with clinical data. An afebrile community control group was composed from a survey performed in a malaria-endemic area. Hb concentrations and anaemia prevalence were analysed according to clinical-epidemiological status and demographic characteristics. In total, 7,831 observations were included. Patients with Plasmodium falciparum infection had lower mean Hb concentrations (10.5 g/dL) followed by P. vivax-infected individuals (12.4 g/dL), community controls (12.8 g/dL) and malaria-negative febrile patients (13.1 g/dL) (p < 0.001). Age, gender and clinical-epidemiological status were strong independent predictors for both outcomes. Amongst malaria-infected individuals, women in the reproductive age had considerably lower Hb concentrations. In this moderate transmission intensity setting, both vivax and falciparum malaria are associated with reduced Hb concentrations and risk of anaemia throughout a wide age range.

Data on chloroquine (CQ)-resistant Plasmodium vivax in Latin America is limited, even with the current research efforts to sustain an efficient malaria control program in all these countries where P. vivax is endemic and where malaria still is a major public health issue. This study estimated in vivo CQ resistance in patients with uncomplicated P. vivax malaria, with use of CQ and primaquine simultaneously, in the Brazilian Amazon. Of a total of 135 enrolled subjects who accomplished the 28-day follow-up, parasitological failure was observed in 7 (5.2%) patients, in whom plasma CQ and desethylchloroquine (DCQ) concentrations were above 100 ng/dl. Univariate analysis showed that previous exposure to malaria and a higher initial mean parasitemia were associated with resistance but not with age or gender. In the multivariate analysis, only high initial parasitemia remained significant. Hemoglobin levels were similar at the beginning of the follow-up and were not associated with parasitemia. However, at day 3 and day 7, hemoglobin levels were significantly lower in patients presenting CQ resistance. The P. vivax dhfr (pvdhfr), pvmrp1, pvmdr1, and pvdhps gene mutations were not related to resistance in this small sample. P. vivax CQ resistance is already a problem in the Brazilian Amazon, which could be to some extent associated with the simultaneous report of anemia triggered by this parasite, a common complication of the disease in most of the areas of endemicity.

BACKGROUND: Approximately 40% of the world's population is at risk for malaria. In highly endemic tropical areas, malaria is a major cause of morbidity and mortality during infancy. There is a complex interrelationship between malaria, malnutrition and intestinal helminths, and this may impair cognitive development in children. The aim of this study was to determine the relationship between malaria and school performance in children living in an endemic area where Plasmodium vivax is the species responsible for most of the cases. METHODS: The study was conducted in the Municipality of Careiro, Amazonas, Brazil, with five to 14 year-old children, studying the first eight grades of public school, during the year 2008. After an initial active case detection, during nine months of follow-up, passive malaria cases detection was instituted, through a thick blood smear performed in every child with fever. School performance was evaluated by the final notes in Mathematics and Portuguese Language. Performance was considered poor when either of the final notes in these disciplines was below the 50th percentile for the respective class and grade. RESULTS: The total number of students followed-up in the cohort was 198. Malarial attacks were reported in 70 (35.4%) of these students, with no cases of severe disease. Plasmodium vivax was detected in 69.2% of the attacks, Plasmodium falciparum in 25.5% and both species in 5.3%. In the multivariate analysis, adjusting for age, mother's education, time living in the study area and school absenteeism, presenting with at least one episode of malaria independently predicted a poor performance at school [OR = 1.91 (1.04-3.54); p = 0.039]. CONCLUSION: Non-severe malaria compromises the school performance of children even during a nine-month follow-up, potentially contributing to the maintenance of underdevelopment in countries endemic for malaria. This is the first evidence of such impact in Latin America, where P. vivax is responsible for the majority of the cases.

Objetivos. Determinar la proporción de admisiones y días de hospitalización inapropiados mediante el protocolo para la evaluación de lo apropiado (AEP) en el Departamento de Medicina del Hospital Nacional Cayetano Heredia (HNCH). Material y métodos: Estudio transversal y descriptivo de corte retrospectivo. Se revisó las historias clínicas de los hospitalizados en el servicio de medicina del HNCH entre noviembre de 1999 y enero del 2000. Se aplicó el AEP para determinar lo apropiado de la admisión, de 2 días de hospitalización aleatorios y del día previo al alta. Se utilizó la prueba de chi cuadrado para encontrar asociación con las características demográficas y de hospitalización, considerando p>0.05 como estadísticamente significativo. Resultados: De 117 historias revisadas, el 5.98 por ciento de las admisiones y el 27.38 por ciento de los días de hospitalización fueron inapropiados. Fueron inapropiados el 48.72 por ciento de los días previos al alta y el 15.98 por ciento de los días aleatorios (p<.05). Se encontró asociación inversa entre admisión por emergencia y lo inapropiado de la hospitalización. Las admisiones inapropiadas y el grupo <65 años generaron mayor proporción de días inapropiados. Conclusión: Los porcentajes de lo inapropiado (admisión y días de hospitalización) en el servicio de medicina del HNCH concuerdan con los valores más bajos reportados en la literatura internacional de países desarrollados. Resaltamos la importancia del comportamiento médico y de la mejor accesibilidad y rapidez en la realización de trámites administrativos para controlar la prolongación innecesaria de las hospitalizaciones, especialmente en los días de hospitalización previos al alta.
